Welcome to Parkside Internal Medicine. Our physicians, clinical and administrative staff thank you for allowing us to provide for your health care needs. We use the latest medical information to deliver quality, compassionate health care to each of our patients.

Our goal is to manage the overall health care of each patient. This includes prevention, diagnosis, treatment of disease and referral to specialists when necessary.

Appointments

We see patients on an appointment basis and request that you contact us in advance so that a time can be reserved for you. Call 317-588-2233 and select option #0 to schedule your appointment. Our clinical staff will ask you a few questions when you call so we may better assist you.

New patients should arrive 15 minutes before their scheduled appointment to complete a medical history form and other necessary information. Patients who arrive more than 10 minutes late may be asked to reschedule. If you need to cancel your appointment, please notify us as soon as possible so that another patient can be seen. Failure to notify us at least 24 hours prior will result in a service charge.

What to Bring

As a new patient, you will need to bring the following items with you to your first appointment:

• New patient packet forms
• Your current medication list
• Your current insurance card(s)
• Medical records from your previous doctor(s)

Billing and Insurance

Parkside Internal Medicine participates in most HMO and PPO plans as well as Medicare. We accept and submit claims to most insurance companies on your behalf. If your insurance contract requires a co-payment for an office visit, this is expected at the time of your visit.

Our staff is dedicated to helping you. Be sure to bring your insurance information and any special insurance forms with you to your visit. It is your responsibility to notify us of any changes in your insurance coverage. Please let us know in advance if you do not have insurance, and we will work with you to develop a plan for payment.
